Before Your First Appointment

To make your visit smooth, please bring:

• Your dental insurance card/information (if applicable)
• A list of current medications
• Any recent dental records or X-rays (if available)
• Completed new patient forms (you can download/fill them in advance or complete them at our office)

Tip: Arrive 10–15 minutes early to allow time to check in and complete paperwork without feeling rushed.
